① PATIENT ORIENTATION BOOKLETS

We have created our own orientation booklet, developed in cooperation with multiple Home Health Agencies and HHA Consultants. It covers CDPH, CMS, and ACHC requirements, and has passed multiple ACHC certification and accreditation surveys.

Our booklet is customized to each and every individual agency. It is professionally printed on 8.5x11” saddle-stitched stock, with a color cover and B&W interior. Seven specific policies are extracted from each agency’s Policies & Procedures manual, and pasted in each agency’s booklet. A list of Local Community Resources is tailored to the agency’s Geographic Service Area. Because of this, the total number of pages depends on some of these customizations, and is generally around 64* pages. The cover is often styled after the agency’s brochure and colors.

We will work with you directly during the customization, and arrange reviews of draft versions via Zoom meetings or PDF files.

* Number of pages depends on the total size of policies exported from you P&P manual, as well as the size of your Geographic Service Area.

② PATIENT ADMISSION FORMS

Just as with our own Patient Orientation Booklet, we have also created our own library of forms for patient admission and patient visits. Most are mandatory, while some are optional depending on your agency’s needs. Most are NCR “carbonless” forms, professionally printed on high quality 8.5x11” NCR stock.

In addition to our customized booklet and form products, we can also “re-print” your existing print materials: while we may not offer full “Graphic Artist” services, we can still professionally re-print your existing print items, provided digital artwork files can be provided.
