The process for customizing your admission packet will start with the collection of various information and several key documents, which include the following:
Complete business information:
Agency basics
Legal business name
Facility name
Address
Business hours, as registered with CDPH
Contact information:
Internal contacts for the project (name, email, phone)
Public contacts for the forms/s/booklets (email, phone)
Names of Administrator and DPCS/DON
Your logo in high-resolution
Documents needed:
Your agency's written Mission Statement
Welcome letter, preferably written from your own perspective
List of specific/detailed services offered
Your Policy & Procedures manual in Word or PDF format *
Your agency brochure
Your Geographic Service Area as registered with CDPH and CMS (list of California counties)
* Policies: Regulations require that seven specific policies be provided to patients. These policies are extracted from each agency's own Policies & Procedures manual, included in the agency's customized Patient handbook, making each Handbook design specific to each agency. Written permission is also required from the publisher of your P&P manual: for Corridor/WellSky manuals this is a simple email process we will walk you through, however please note that at this time we do not support Med-Pass P&P manuals. The required policies are:

Process start: Once you are ready to start this process and have all of the required information and documents at hand, please complete our new client onboarding questionnaire here. After completion of the customization process, we will submit "review draft" PDF versions for your review and approval.
Folders: for custom-printed folders, we typically use the design/color scheme of the agency's existing brochure to design a custom folder. For this process, we need a PDF of the brochure's design that is an original, not a scan or photo.
For agencies that are still in their start-up period (waiting for their licensure and/or accreditation surveys), the review draft versions of forms and booklet are generally sufficient for the purposes of both surveys, even though they have a "review draft" watermark. The agency can print a copy of these drafts and present for both surveys. ACHC accepts watermarked draft copies during both of their surveys. Store-bought folders can also be used during this period, if the agency will be ordering customized folders.
Once the agency passes both surveys, receive their CDPH Readiness Letter, and is ready to admit their first 10 patients, two options are available for packets of the first 10 admissions:
Print a minimal quantity of forms and booklets (10 to 12)
Print a production quantity of forms and booklets (100+)
Folders can again be store-bought (custom-printed is 250 minimum)
