Forms & Booklet Customization Process


What We'll Need From You

The process begins with collecting various information and key documents. Please have the following ready when you're ready to start:

Business Information

  • Legal business name
  • Facility name (if different)
  • Address
  • Business hours, as registered with CDPH

Contact Information

  • Internal contacts for the project
  • Public contacts for the forms/booklets
  • Names of Administrator and DPCS/DON

Logo

  • Your logo in high resolution

Documents Required

  • Your agency's written Mission Statement
  • Welcome letter (preferably written from your own personal perspective)
  • List of specific/detailed services offered
  • Policies & Procedures manual *
  • Your agency brochure
  • Geographic Service Area as registered with CDPH and CMS (list of California counties)
* About Policies: Regulations require that seven specific policies be provided to patients. Seven “general summaries” in the Patient Orientation Booklet convey the underlying intent of these policies and their key requirements. Each summary directly references the exact policy in each agency’s P&P manual by name and policy number. Each agency’s P&P needs to be carefully reviewed to correctly identify these seven policies and ensure proper references. Each agency’s P&P manual is treated with strict confidence.

How the Process Works

Gather Your Information & Documents

Collect all business information, contact details, logo, and required documents listed above before starting the process.

Complete the Onboarding Questionnaire

Once you have everything ready, complete our new client onboarding questionnaire: → start the questionnaire

Customization & Review

We customize your forms and patient orientation booklet, then submit "review draft" PDF versions for your review and approval — typically via Zoom meetings or PDF file review.

Approval & Print

Once approved, your materials go to print. Small quantities of forms (10–12 pieces) can be printed in-house for faster turnaround; booklets and production quantities are sent to our print partners.

For custom-printed folders, we typically use the design and color scheme of your existing brochure. For this we need a PDF of the brochure's design that is an original — not a scan or photo.


New California HHAs in Their Start-Up Phase

For agencies waiting for their licensure survey, our “review draft” versions of forms and booklets have been accepted by surveyors (but always verify with your accreditation agency). You can print a copy of these drafts and submit for the survey. Store-bought plain folders can also be used during this phase.

Once you pass the licensure survey, receive your CDPH Readiness Letter, and are ready to admit your first 10 patients, we can go to print with either minimal or production quantities:

Minimal Quantity

Print 10–12 copies of forms and booklets for the first admissions. Faster turnaround, higher per-unit cost.

Production Quantity

Print 100+ copies for ongoing use. Better per-unit pricing, suitable for established workflow.

Option: Custom folders

Store-bought folders are fine during start-up. Custom-printed folders have a 250-piece minimum.
